Requests For 2011-2012 will be accepted on a first come first served basis beginning
June 15, 2011

Please complete all information and return to the Gym Sask office by email or mail with sanction fee of $40.00 (includes SOCAN fee). Request by email must be accompanied with a credit card payment. Sanction fees paid by cheque will be considered when cheque is received.

Sanction requests without payment will not be considered, deadline for requests is August 1, 2011.

Sanction fees will not be refunded to clubs that withdraw their application or cancel their event.
